I hereby acknowledge and understand that Brazilian Jiu-Jitsu is a physical activity and learning self-defense skills involves some physical contact. Leviathan Brazilian Jiu-Jitsu Academy or it's staff and students are not to be held liable or responsible for any risks and/or injuries as a result of training Brazilian Jiu-Jitsu.

The people stated above hereby hold Leviathan Brazilian Jiu-Jitsu Academy, it's successors, assigns, owners, officers, directors, employees, and agents harmless from all claims which may be brought against them by the people stated in this contract, student or on student's behalf for any such injuries or claims and do by hereby forever release and discharge, it's successors, assigns, owners, officers, directors, employees, and agents from all claims, demands, injuries, death, damages, actions, losses and expenses.

Leviathan is a privately owned business, we retain the right to deny any potential student or cancel any exiting membership at any time without notice or reason at our sole discretion.

PARENT/GUARDIAN CONSENT: The above said person is the legal parent or guardian of the student participating in this introductory. The above person consents to the student participating in the classes/training and are aware of the risks involved in such participation.

By entering into the agreement the undersigned grants the school permission to use photographs, videotapes, artwork or other likenesses of the student for marketing, trade, publishing or any other lawful purpose.

In consideration of the martial arts training obtained, student agrees not to engage in martial arts teaching practices within a 25 mile radius of the school, or any other school associated with the schools administrators, for a period of 2 years following disassociation with the school, without written permission.
